It exists, it's the YH-5M.  More of canal phone than IEM, but canal phones are so much more comfortable. 
 
I wonder if Stax 003 style drivers could be revised to be an ortho instead of 'stat.  Not willing to sacrifice mine to the cause though.

Yeah, I've had some "designs" on the 5M's myself.

Yeah, pretty much like the YH-5M with a simpler headband. The probable reason for the 5M's complexity is to make sure the little canal tubes are aimed precisely so that a perfect seal is established as soon as the owner puts them on, since a perfect seal is of course essential for bass.
 
A 5M was rumored to have been pulled out of the sky the way Willie Mays did it, and by one of our own HFers. I wonder whatever happened to that, hmmmm?

Ok so not sure any of you noticed this before but, it seems Elega and Akai are bedfellows. I discovered this when I opened an Akai electret energizer and the PCB read Elega, then I looked at the Akai/Elega orthos and discovered they look the same.
 
Akai ASE-50 and ELEGA FR-901C seem the same:

8Ohm driver update:
 
The 8ohm ortho turns out to be only slightly more easy to drive than the yhd2 and is being a pia to damp easily with short time. It sounds pretty good directly out of my laptop but not from the DAP. I prefer that coloured midrange from the yhd to the stark reality of the SFIish driver. I mean, if I wanted a small grado they literally give the iGrado away. It has been boxed away until I have time to play with them again.

Service announcement: I just finished updating the search engine to reflect the recent changes on HF, and now the index is updated to today's posts, and will hopefully stay updated in the future. One problem though is with image attachments: HF uses a braindead URL scheme for attachments, which I'm not able to fetch with the current code unless I make major modifications, so image attachments on HF will not get stored and indexed.

On the ortho front, I still have to unpack a pair of YH100 I got from ebay two months ago. :p
