For Spiral Dots I would probably say the medium size is better compared to my medium red noble tips. The medium Spirals are about 1mm smaller than the Medium Nobles but the Spiral Dots are more flexible compared to the red tips which I always thought were a bit too stiff for my ears. I haven't tested the Medium-Large (ML) Spiral Dots simply because I don't own those but I would surmise they will be a few mm bigger than the Spiral Dot M and therefore may be a bit too large and uncomfortable. YMMV however.
