Oh well - I wanted IEMs for a lot of reasons
* The look
* I wanted to hang them off a Sony LDAC neckband for wireless Hi Res
* Noise isolation
I bet the ER4 XR'S sound sweet - if only they fit me.

You could try iems with over ear cables such as westone and all the other chinese models.
Simpler solution though, Have you tried a clip to secure the cable to your shirt?

The shure ear bud gets about half a millimetre into my ear canal. Then falls out
