I think Kozee Sound Solutions has the cheapest remolding prices and the price includes a cable.
 
edit: It also includes a free do it yourself ear impression kit. An incredible value.

Kozee and Fisher are the same price for reshelling and cables (90 + 30). I think Average Joe confirmed cables aren't included, just that the detachable cable connection is included with acrylic reshell but not with silicone reshell (it is confusing sounding on the web site); but the impression kit could save you anywhere from $20 to $100 depending on audiologists in your area.
