My understanding is that hiss is increasing on higher gain. Therefore low gain for iem's.
But on headphones? No big differences i think, so i stay with the lowest gain with enough volume

Does sound signature change on medium gain?

I use medium gain because I think it sounds better than low gain.  I find high gain is too fatiguing.  Don't be afraid to experiment and use whatever setting sounds best to YOU. 
 
Just remember that if you play IEMs too loud on ANY setting, you'll damage your hearing.

I always keep the sound at a reasonable volume. My hearing is too precious to me.

I've been using QP1R with SE846 as well.
And even at low gain and volume level of 20, hissing sound is noticeable on standby mode.
Albeit, when the music starts the hiss is almost gone.
 
Is it only my device?

It is normal. Probably more obvious with sensitive iems. But heard from others v2 has lower noise floor, so not that apparent or gone.

I received one of the newer units with the improved click wheel and it seems fine. several recordings have a little hiss, but I've determined it's in the actual recording, because really, really super well recorded albums like Michael Jackson or Dire Straits have zero hiss

People who are more sensitive to hiss will hear it with the QP1R and SE846.  Is it not a problem with your device or your IEMs.

QP1R on high gain with Audio Technica is an excellent pairing.


Funny how all of our ears hear differently. High gain with my Audio-Technica ATH-MSR7GMs is shrill and fatiguing. Mid gain is heavenly for me.
