I don' think you should be able to hear a person next to you at a moderate volume if the fit is good. You should have a bit of discomfort initially when inserted, but then that should go away after a while.   More importantly, the bass should be there. If you don't have good seal, you cannot...

Initial discomfort may be OK, but there shouldn't be any pain. I also suggest leaving them in for an extended period (1+ hour) and if they are uncomfortable enough for you to say "Sound-be-damned, I need this out of my ears!"  If so, then you should get them refitted. You are definitely aware...
