These are just like any other custom molds, fit them like any customs. The only reason that these look odd is how it was laid down and then photographed. If these are not an impression of yours, then you have get them remolded.

A lot of companies are going for the super slim custom now. Quite sweet. If they were made for your ears, just screw them in.

My guess (and it is a guess) is that it isn't a wax remover but a thingy to pull on to get them out of the ear as they don't look like full size so would be difficault to get hold off

If you mean that pin like thing on both sides then yes it is not a ear wax remover but something you can use to pull out of your ears should it ever be needed. Also from the first pic it looks more like the molds you put over the body of the earphone than anything else.
