I clean and feed my pads to stop any damage to them, but I’ve never seen that before so obviously don’t suffer with it.

Yes, this. Especially with the perforated, more delicate ones that come with the Utopia. The Stellia pads I swapped with are a lot more robust so I’m hoping they hold up.I think sweat penetrate earpads and glue become lose to wreak havoc.
Same thing happened to my hd800S headband.

Let’s say Utopia has a high failure rate 1 out of 10 - yours failed 4 times. The probability is 0.10^4 = 0.0001 or 1 in 10,000. Not outside the realm of possibility of being unlikely but I’ll bet it’s not a faulty product.
