I believe it is when you take the stock triple flange, and cut off one flange from either end.

It basically make a triple flange into a double flange. Only the ER6 series
have true double flanges available from Etymotic

That's what I was figuring but since the tip stems are different lengths you almost have to estimate how far to put it on the nozzle. I just got the bi-flanges. I love them.

ER-6i owner here. I'm hopefully going to upgrade to ER-4P within a few weeks because of a demo at macworld.

I plan on pairing this with an LDM+ and 1g nano. I'm also considering a custom ear molds down the line, and would like to hear more from people who've had it done. I always wanted custom fitted IEMs, but didn't want to break the bank, especially since this is only going to be for a DAP. Are other accessories/modifcations I should consider like that p > s adapter I read about, and what kind of changes should I expect.

Oooh, me too (ER4p with p->s adapter). I even put them up against the K1000 except in the extreme highs.

In P mode: Triple flanges (or when I expect to have to take them in and out often)
In S mode: Comply tips

It's essentially just a slightly reduced foamy tube jammed into a triflange with the last nub hacked off.
