An elderly man -- a retired Greek chef, once a master of his trade -- travelling in Morocco is admitted to the ER of a hospital, complaining of chronic pain, debilitating migraines, and insomnia.
 
The head doctor that evening is a pleasant man of Persian descent, who prefers to get to know patients a bit before treating them.  His experience has shown that building rapport with the patient, and gathering some background information on them, can actually prove vital to saving lives and improving their condition.
 
After taking a look at this man's charts and intake paperwork, the doctor walks over to him and asks genuinely, "Tell me about yourself. What brings you here tonight? How do you feel?"
 
The Greek man, coughing and straining to speak, says, "I falafel"

Long build up to a goofy joke.
 
How about, What did the Greek restaurant owner say to his doctor? "I falafel"
 
I think that's about all the prepping that joke needs.
