I'm sending in my SR325is to be fixed today, which is not a business day so they aren't available to ask. I figured someone here would know. On the return form it says "Model Number" so would that be "SR325is" or the number written inside the box? Also, I assume "Cartridge Number" is referring to their record player components and not needed here, right?

Yeah it would be that, they're not serialized so that would definitely be it haha...
 
Either way they'll know it's just for the billing department to determine how much to charge you...
