Out pin is the virtual gnd. Com goes to V- In goes to V+. The center pin of the TLE should be connected to V- not gnd.
Layout is starting to look better. The side of the board by the output jack is significantly better than your last layout. I like how you've got all the resistor s lined up so the vgnd trace runs in a line. See if you can sort out the resistors on the other side. Sometimes just rotating a part or maybe swapping two left to right will help a lot.
The autorouter isn't helping things. Look at the path from R2R to capR. Try ripping up a few traces and routing them by hand. You can also have Eagle auto a single net.
