like on the default ipod earbuds?
do any over-the-ear headphones use the tiny mini plug?

Plenty of them come with the 3.5mm plug and a screw-on 1/4" adapter.

Yes many. Even the HD580 and (AFAIK) the HD650 use a mini plug.

Did you mean if they come with the 3.5mm plug that's thin enough to fit in an iPhone jack? If so, basically none of them do, save for those made specifically for it. You don't want to go there, though.
