DAC/Pre -> Headphone Amp ?

Many DAC's also offer the ability to be a preamplifier. For example: Berkeley Alpha, Wyred4Sound DAC-2, etc.
 
Given a separate DAC/Pre and separate headphone amplifier; How would one control the volume of a dedicated headphone amplifier using the DAC's volume control? 
As every dedicated headphone amplifier has it's own volume control, do you just need to set the headphone amplifier's volume control at it's highest setting, then control the volume with the DAC/Pre?  If the DAC/Pre has remote control capabilities then it would be handy to be able to control the volume with the remote.
 
Has anyone ever made a dedicated headphone amplifier without an integrated volume control, just like a separate stereo amplifier, but with headphone outputs instead of speaker outputs?  To then have the volume controlled by a dedicated preamplifier?
