I seem to have large-aperture ear canals, and have to use the biggest diameter tips in any assortment.  Usually large Complys work, but in some cases even those do not give me the best seal.  So....
 
What's the widest good tip you know of - what's actually worked for you, fellow big-ear 

   Bore is relevant as well, but I am curious where to go to augment my tip arsenal for the ocassional uncooperative iem.

Hi, I also have that problem, and for me what works is fitting a smaller tip inside the largest one.
you can also use some foam and make a ring with with, then place it around the core tip.
 
That's how I usually solve the issues when I want proper seal.

I throw a pair of large triple flanges on most of my iem and these always seem to do the trick.

Im in there with you, and my right ear is tricky requiring even more effort. The super huge Comply tips work, I don't think the triple flanges will seal my ears. Oddly the Ety foam will seal when seated so deep you cannot see anything but the cord exiting my ear with the HF5's. 
 
I usually use Sony Hybrids size LL. If they don't the nozzle (most BA's) I usually de-core the smallest sized foamy thats included with the IEM and press the core inside the hybrids.
 
Big ears make housings fitting a breeze, but they keep me tip rolling for usually a month after a new purchase -_-

I'm part of this group, even the largest of tips won't seal and basically slide out for me which is incredibly annoying most of the time it's like we need one extra size up. I know of two options which work for me. I'll post some links in a few hours when at a computer.

My ears are "large-sized" too.
Sony Hybrids, Shure and Westone Tips, Shure Olives (Foam) fits my ears perfect.

Also Monster Tips has a lot of units with a large size (+ Supertips, + Foam)

Same problem. The large Comply foam tips barely provide a seal, and I shouldn't move too much! Actually, their "active" model doesn't even seal at all.
 
The Shure black foam "olive" tips work better, but not perfectly -- still too small.

Monster Supertips are your last chance, I've never seen a bigger tip. They're rather expensive remembering only one of each type will fit.




I wish we could come up with a better way to fill out the inside of the tip like people are doing with foam, because it's mostly the problem, if we could work out a good method to pack out the tip we'd get much chance of achieving a seal. I've thought about it over and tried from chemist foam ear plugs to cutting out foam rings but nothing really satisfies me.

This is the mod I'm talking about, we need a better way of doing this, something a little more professional.

I'll keep them in mind ... but not order them now, because they wouldn't fit the earphones I use most on the go (Bluebuds X) and I have customs (4.A) for home usage.

I've already used this method with my Bluebuds X tips. It does help expand them, though, of course, there's a limit as to how much they can be expanded. I'll see if I can get enough light to take a picture.

The problem for me is the tips tend to collapse once inside the canal, so they never really grip and slide out, so something like filling them out with foam would do the trick (your canals must be huge 

) I was tinkering around with my tip collection and found one particular tip that slides nicely inside the one I want to use filling out the gap. But to do this you cut a majority off the stem of first. I'll make a step by step guide up when I have time. It's pretty much the same as using foam like you have but placing another large tip inside the one you want to use after cutting 3/4 of the stem off, cutting the stem allows the 'stem' of the tip you want to use  to stick through easily. 

It seems to be working but we also need to consider the sound changes for example although it sealed for me I think there was more sibilance. 

You know it's been driving me crazy for many months now, I search on eBay thinking of things to try. The endless search for that extra size up tip.

Btw: Those Monster Supertips come with little red adapters so they fit Shure, Westone and the likes.
