a. fit for high impedance or low sensitivity headphones
b. fit for medium impedance or low sensitivity headphones

I have a LDIII and I only use the lowest gain setting which is 3 on mine. (both switches on)
The manual that came with mine completely contradicts the newer version of the manual on LD's forums.
Just try the higher settings and you'll find it increases bloom and makes the bass muddy. Try a gain of 10 and you'll hear the sound really isn't good anymore.
Then come back to a gain of 3 and you'll find the sound much more enjoyable. I tried it with all of my headphones and found the lowest gain always the best.

High sensitivity, low impedance cans need amps with high current rather than high voltage.  Whatever setting provides the highest current is the one you should use.  But ultimately, I think whatever setting sounds the best is the one you should use.
