My bad; I forgot about the inline volume controller. Unlike most headphones with this type of controller, where it changes the volume on the iPhone, this one is an attenuator. 

I just bought the MX 585, because don't like in-canal earbuds. I wanted them to listen on my iPhone, but the volume is incredibly low even when the iPhone is maxed out. Is this inherent in these newer earbuds? I never had this problem with Apple earbuds back in the day.

I've been reading reviews, and the AD2000 is out of my price range; but what about the AD900?   I'd be worried about the Grados not fitting well. I have large ears, and I really like the amount of space on the HD580; the AD900 looks to be similar, but the Grados look quite small.

No, my source is clean; with other headphones, there's no noise.   Altering the volume did alter the level of the hiss, but at normal volume, the hiss was notable.
