Depending on mastering level I hover between 1/4 to 1/2 volume (Maximum 1/2 is usually too much) for most albums. I only use the option for leveling the sound between albums when I'm shuffling songs on playlists etc.

+ 1 that's what I'd try as well, also Sony mentions in the manual to put the SD card into the player first and then load it with music. You may want to wipe the card and then re-load it with music to see if that helps. Just a shot in the dark

It doesn't support true balanced output, it does support a 3.5 TRRS jack for separate grounds though. Sony states it's a minor improvement in separation and crosstalk. I'm getting my aftermarket IEM cable re-terminated as we speak, I'll be sure to post my impressions when I get it back.
