Yes, I cut the end off the Grado 15 ft extension and it has 4 wires.

Use the calibrated volume control setting if your amplifier has a volume control. The calibrated setting is for line out adjustment. The variable setting is used if the amplifier does not have a separate volume control. You can also try both ways and see if you can tell which sounds better.

It is a 5 pin XLR connector because I wanted a ground pin on the amp jack to also support unbalanced headphones without having to add an unbalanced jack.

I cut the TRS plug off my SR225 and RS1. They both have four wires. I put a 5 pin XLR connector on them for balanced connection to my amps. I think all Grados have four wires to the plug.
