I was one of the people DJScope mentioned who perforated their eardrum from removing an ER4S.
Its a very painful experience, takes months to heal and you can easily reoccur the damage simply by sneezing with your mouth closed during the healing process. I was partially healed, had to sneeze one night in bed, didn't open my mouth and the pressure tore my eardrum once again putting me back basically near 2-months of healing process, air came rushing out my ear. I had to be so careful anytime I sneezed to release the pressure for months on. (and still do)
When people say you don't forget the first experience of some pain for example a dentist drill hitting a nerve, perforating your eardrum holds the same theory, just think about it for a second the pain comes rushing back, you never forget it.
Always try and pull down on the IEM to break seal before removal, though in the case of ER4S its a very tight/deep seal with triple flange tips. Little different from regular tips.
