Your toughest customer

Okay. I know this may not exist but I'm hoping you guys can lead me to the best/closest headphone out there to these specifications-
 
A little background - These headphones are going to be a gift to my mom. She doesn't have too much (any) experience with hi-fi audio/audio equipment, but I'm out to blow her socks off. She will be listening out of a iPhone 3GS, and possibly upgrading to a 4S in the future. Finally, she is almost deaf in her left ear (pretty important). All that said, can you help me find a pair with these specs:
 
-$200 budget, but I can be loose if I find the golden ticket.
-Has a microphone
-NOT over the head/full sized OR ear buds OR ear clips. Ear buds don't fit my family's ears, and shes just against full sized ones for some reason. I think in-ears would be optimal for her, but I'm open to hear suggestions. Ear clips look awkward on us, but if you think they might not (super adjustable ones) please suggest it.
 
*Extra credit - Here's the tough part - If possible, I'd like for her to have the ability to adjust the right/left balance levels somehow through the headphones themselves. I've only ever seen this on like one wireless sennheiser, but I'm putting it out there if anyone has any ideas or suggestions as well.

Yould could get IEMs with replaceable cables and DIY a volume pot on the right side to attenuate the volume...

Shures, they have replaceable cables, and you can buy the mic and remote separately. Hope this help :wink:

Wow this seems like a great start. How can I DIY a volume pot? I'm not too savvy with these things. Every bit of help will... help!

I would look for earphones where the cables are detachable at the Y-split rather than (or as well as) at the earpiece.  That way the DIYed volume pot will be much more handy.
