I will only buy the "ciem-like" shapes as they are the only ones that have ever been comfortable in my ears. And if I cant wear it for 8 hours straight without my ears hurting then it gets sent back.
The angle and depth of the nozzle section needs to be correct too though, most dont extend far enough and are a few degrees off. The Mangird (now Xenns) Tea IEMs have been the most comfortable so far. Get some of those so you can see the design and angle they use and model some new IEMs off that.
