If you've had two failures like this with two separate units, I'd wonder if there is a problem with your AC. A surge across the line could conceivably take out the Schottky bridge. Because we haven't seen excessive failures of this product, and two in one location is suspicious.
I do not know if the problem is the resiter R49 or the small thing B20 100 2151 at its left cause the problem.....

I would take @Jason Stoddard ’s voltage comment seriously. Voltage surges do occur and they wipe out many components.The voltage mains is stable at the time the MM2 fails, everything are working fine without problem.
Only one thing is suspicious to me, the MM2 is connected with iphone Camera kit 3, and with lightning charging cable, but seems it doesn't related to the power supply of the MM2, I did measured the voltage of the wallwart, it is 17.3V AC unloaded.If you've had two failures like this with two separate units, I'd wonder if there is a problem with your AC. Philips ECG tubes do have a value. If you're helping a friend decide between a tube amp and a transistor amp, throw some ECG's in the tube amp and they'll immediately select the transistor amp.And throw in some of his favorite tubes? Doesn't he prefer the Jan Phillips ECG variety? Someone can correct me if I'm wrong
