I was thinking, the Little Dot MK V is almost a perfectly balanced amp, except for the output (1/4) and the inputs (RCA). I don't have enough know-how to do a mod myself, but I thought maybe I could get a conversation started. Is this mod even possible and/or practical?

You would need two of them for balanced operation. Each channel needs an amplifier for the + signal and one for the - signal in balanced operation.

Would the easiest way be to terminate the headphone into balanced dual 1/4" plug, and have a 3 pin XLR to dual RCA cable to hook up the source to the amp, and leave the amp untouched?

Yeah, you probably could do something like that. Use one amp for R chan, another for L. You would have sep. volume controls, so could control the balance as well that way.
