Listening to your headphones while driving your car

I do it everyday here in Saigon to keep me sane even though the traffic here is amongst the most chaotic in the world, but I must note that I'm driving a moped.

The constant din of traffic noise is its own form of torture.

As posted above, the laws for each state in US vary for headphones while driving, from not allowed, allowed or only one ear blocked.  I've used my custom IEM's in both ears for street driving here in California on my lunch break to pick up food, which is technically not allowed, and it can feel a bit unnerving as sound helps you with locating vehicles in your blind spots.

Open headphones don't shut out the outside noise completely, so you can hear sirens and such.  Is it a matter of concentration?
 
I think driving a motorcycle is much too dangerous as it is and adding headphones would be hazardous.
