Whats your budget? Etymotics and some of the small final audio balanced armature headphones might be worth thinking about!

If isolation isn't a hard requirements, earbuds might also be a good option.
Also worth experimenting with different types/shapes of tips, although that's hard advice to incorporate into a gift : )

I have small ear canals and find small over ear sets best. Like the Brainwavz B150. As above the Final e2000 might be a good set. I have the more expensive e4000 and they're a great fit. The 4000 has quality sound and I've heard good things of the e2000.

I think the Final e2000 would make a good gift as they are very modern looking and if they're anything like the e4000 in terms of sound quality and packaging I'm sure your friend would enjoy them. Without being disrespectful to cocolinho I'd stay away from the re-400 as they have a reputation for breaking easy - one of the housings on mine split apart after 6 months. I'm not the only one with QC issues. Plus sound wise they are very neutral and analytical.
Like I said, the Brainwavz b150 are very comfy and sound great, nice bass and wide sound signature. If your friend has issues with IEMs then maybe a change of tip... like SpinFits. They go deeper in to the ear canal.
