The word 'extra' exists in French...so you could say 'extra petit....'

Or, 'en plus petit,' or 'de plus petit' depending on use.

Haha, "très petit", that's french but it doesn't sound very good.
If you're talking about something in a conversation, you can use "tout petit", or "minuscule" (synonym for miniature, but miniature is only used for scale models).
But if you're about talking about shirt sizes, we use the same letters as you : XS, S, M, L, XL. No french equivalent. And I have never seen "T.Grand", you may ask the salesperson for a "très grand t-shirt" because you only know you need a very large size but don't know the exact size, and he/she will give you an XL size.

Of course not, but personnally, being a 22 year old french (born and raised in Paris), I would say "c'est très petit", but would tend to avoid saying "c'est une très petite voiture". That's perfect french, but the sentence is just not very beautiful.
