I wash my tri-flanges in warm soapy water after each use and replace them every month or two. Each use means after every trip on the plane (which averages about 2-3 hours use each).

I'm still using the same Er-6 flanges after two years. I just wipe them down once in a while, but really, they never get grungy. A good wiping with a paper towel does the trick. My ears are pretty clean, anyway, and I haven't yet had to replace the filters. I do regularly clean out the passage in the flanges, though--wax is natural in your ears, but not in your phones!

I just started using foam plugs with my Er-4s, and they do pick up some color. Again, colored is distinctly different from dangerous, but I'll probably be inclined to wash them every couple of days, since color buildup is an indication that stuff is actually sticking.

Unless you have holes in your eardrums, I suspect fear of infection in this topic is mainly hysteria. Doctors, what say you?
