Does anybody else use a different sized ear tip in each ear? Sounds goofy at first but I would think this is much more common than one would think. Sort of like my ring on my ring finger won't even go over my knuckle on the same finger of the other hand.

I guess this could be a large reason if you are getting an unbalanced sound.

Yes, I have in the past. My left ear canal is slightly smaller than my right. I used to use the white Ety tip on my left and the grey one on the right. But with the Shure or Comply tips I don't do this any longer.

yes and its very annoying, the only suitable way I found to get around it was custom tips

Yes. Humans are not symmetrical. I use the largest tips on my Sennheisers (I am a big man) but the left side is still loose compared to the right. Owls even have one ear canal higher than the other to help them localize sounds at night.

I have the same problem. On my right ear the large tip of on my CX300 is perfect, but on my left ear the large tip is just a bit too big and it will start to hurt after more than an hour. The medium sized tip is actually just slightly too small for my left ear though, which means I'll just have to make do with the large tip.

I just can't wait till my RE0s arrive, as I hope they will have the right tip size. I can only hope though.

my right ear is noticeably smaller than my left ear. pretty annoying at times finding a correct fit with silicone tips, a lot of times the largest tip would be good for my left ear but would be too large for my right ear but then the medium tip would be too small for my right ear. i ordered some foams which should help fix the problem since they can conform more.

The key, I suppose, is to get a huge selection of tips and work with that.
I have the large Sennheiser single flange for my left, and the medium UE single flange for my right ear (both stuffed with Complys).
