I plan on driving my SR-60s with this CMoy, and I'm probably going to stuff it in an altoids tin (which is really appealing to me).

1. Should I choose different gains?

2. Do I need an R5?

3. Is this the best opamp?

1) Change R4, not R3, there's logic behind that. Gain of 4 or 5 is good.
2) Likely not unless your source sucks
3) Nope, check out OPA2134, OPA2604

Unless you need the input caps, why not just leave them out, you'll have better sound quality. Or perhaps swap it for a 1K resistor.

Of the chips we've discussed so far, none are any more at risk of causing problems than the recommended OPA2132. They're all docile.
