I've used Raychem boots for Y-cables. They are available in various sizes. Some even have an epoxy lining, which allows you to make an environmentally sealed cable. (Important for what I do, but likely not for most DIY'ers) Check the usual online sources for availability. It's been a while since I've used them, so I'm not sure where to buy them.

Cheers,

The easiest and probably cheapest way other then to get thoes "special" ones is to simply buy resin filled heatshrink, slightly larger then the cable in question.

Put it over, heat it up and while still burning hot grab some pliers and pinch the Y split. Instant heatshrink pants!
