Where to get custom tips for universal IEMs?

I'm looking to get better isolation and fit out of my IEMs. I've used a pair of etymotic er-4p for the last 2-3 years, and I found that I need very deep angled insertion in both my ears to get a proper seal. I bought a set of fischer audio dba-02s, but couldn't insert them deep enough to isolate properly. The same thing happened with my new pair of miles davis tributes. It just seems like my ear canals are quite oddly shaped until I push the phones pretty deep, however only the er-4p are slim and straight and will let me do so, the other ones push on my tragus and the seal is very hard to keep for more than a few seconds without having to hold everything by hand.
 
Now, I'm considering buying some custom tips that could fit onto most universals. Even though there are lots of custom IEM makers, there doesn't seem to be a lot of choices for custom tips. Who offers good quality products, especially with DIY impression kits (audiologists are expensive!)?
