There are so many great ones but
Bob Dylan - Bob Dylan
is definitely one of them.

Opiate was before Undertow.

I suppose that's true, but it wasn't really album length. Wasn't it all live?

There are only two songs on Opiate that are live. With the exception of "Part of Me," none of the songs on Opiate appear on any other album, which is why I consider it a legitimate album.

NaS - Illmatic
Catch-22 - Keasbey Nights
Lupe Fiasco - Food and Liquor
Snoop Dogg - Doggystyle
Streetlight Manifesto - Everything Goes Numb
Dead Kennedys - Fresh Fruit for Rotting Vegetables
Operation Ivy - Energy
Bad Brains - Bad Brains
Dr. Dre - The Chronic
The Specials - Specials

I know that is a lot more than one album. Whatever.

Wynton Marsalis - Wynton Marsalis. Young Wynton; young Branford; simply awesome straight-ahead Jazz. The cut, "Sister Cheryl" became one of my all-time favs.

Put out another or two great recordings and went downhill from there in my opinion.

Beck - Mellow Gold

Bruce Springsteen - The Wild, The Innocent, The E-Street Shuffle (Some feel that Born to Run simply blows this one out of the water, but I think this is his most personal work. Just has a great atmosphere and mood, can't get enough of it. This post actually motivated me to go put it on.)

Brian Eno - Here Come the Warm Jets (Does this count?)

The Pixies - Surfer Rosa (Unless you count Come on Pilgrim)

Jesus and Mary Chain - Psycocandy

King Crimson - In the Court of the Crimson King

Some of my favorites I guess.

Those are your favorite first albums, I understand that..but not sure that any of them could be THE BEST first album of all time, could they?

Sure, why not? First solo album, wasn't? And it is one heck of an album.
