Weird EMU1212m analog board problems

Has anyone had an EMU1212m where one of the phases of one of the channels on the analog IO board is much quieter in volume than the others? Mine has always experienced this problem as far as I know, I just used to ignore it and use the inverse phase of both channels (I use a mono adapter so I use it unbalanced) by pulling the mono 1/4" out slightly so the tip contacts the sleeve of the jack. The "quiet" right channel + signal also has a 10.0 mv dc offset constantly.

Today I received my LM4562s and went to work to replace the JRC2068s on the outputs, thinking that it might have been the problem.. since the DAC is cap coupled to the output opamps. The replacement worked fine and they sound good but there is still the offset and less volume on the positive output of the right channel. Anyone have any suggestions? It would be logical to look at the coupling caps and I'm pretty sure I replaced them at one time and it had no effect but it was about a year ago and I can't remember.
