I'm having trouble finding a splitter that meets these requirements:
 
1 4-pin XLR balanced cable input
 
1 4-pin XLR balanced cable output
1 single ended 1/4" cable output
 
 
I'm trying to have my amp output to my HD 800 (with balanced cable) and my Subpac S2, and I believe a splitter or other box of this nature is what I need.
 
Does anyone know of something that could fit this requirement?

Y-cable splitter is not a good idea. You may damage your amp by connecting a SE device to the balanced output.

I have never used Subpac but it appears it has its own amp. You should be fine splitting SE signal from the DAC or using SE output from your amp if it has one and it can be used together with the balanced one. Otherwise you may need a separate preamp or something like that. But this path would be rather expensive...
