NJM4556 opamp (29 cents)
100k pot
100k input resistor
121k and 464k gain resistors
5uF input coupling cap
.12uF power supply bypass caps
2x 9V batteries hooked in series with "center tap" grounded
And thats it. Get some RCA inputs, a switch, a volume knob, and an output 1/4" jack.
