Question about IEM sound stage and surround sound

First post.

I've searched the forum and Google, but can't seem to find an answer on this. Can IEM's pick up the direction of where sounds are coming from in a game (FPS for example)?

I understand that IEM's generally have a poor sound stage, but would it still be possible to know if the sound is coming from in front of me, behind me, to the left, etc. I'm not interested in virtual surround sound.

I'm looking to purchase IEM's, not for gaming, but I'm wondering if it could serve that purpose. Some of the ones I'm looking at aren't intended to be worn around the ear, but could be as an alternative which is what I prefer. This rotates the earphone so the logo usually sits up or down. Are IEM drivers positioned in a way where there is a front and back? Or is wearing them either way have no effect at all?
