Does the PSU that you have include a polarity switch?
If the LED powers up, this doesn't sound likely, but imagine your dismay, if you send the unit back to JMT... he diagnoses, and fixes whatever problem your unit may have... you get it back, plug it in to the wallwart... but the polarity is inverted, and blows the new op-amp straight away???
And, one final thing... which might sound incredibly insulting to you, but that should be checked... 12v @ 1amp... we are talking DC here aren't we? because I've only ever seen AC PSUs that push out that kind of current, and that'd be a HUGE no-no if that were the case
