That's odd.  I saw a form ... and when I liked the page, the form disappeared.  So I unliked the page, filled in the form and liked it again.
 
But does that mean I'm not entered because I didn't see the real entry form?

Wow, thanks for the heads up.  When I unliked the page, the form showed up.  I did the same as you, filled it in, and re-liked the page.  Must be a bug with facebooks program.

For those asking for a screenshot, here's basically what it looked like for me when I unliked the page. Using Chrome v30.0.1599.69, the form comes up immediately once I hit unlike.

This NEVER came up for me, no matter what browser I used (chrome, firefox, IE, opera), or whether I used a mac, HP or my Dell at work. Even un-liking and iking it again didn't cause it to show up. I contacted Moon Audio on facebook and they filled out all my info themselves, they think. Hopefully they did.

Hmm, that's strange that everyone was having a bunch of issues...
I was able to use the latest Chrome Beta build on my Nexus 4 to enter the survey while in mobile and not desktop mode.
