Making a headphone run from power-amp speaker outs

I've got an old NAD 2600 amp (not an integrated) that I haven't used for number of years, that I would like to use to run headphones. It has no headphone out jack but does have volume controls on each channel. Does anyone know how to wire a haedphone adapter to run from the speaker outputs? They have both pos and neg for the left and right channels, so require four connections for output. I tried a rudimentary rewire of a radio shack headphone extension by cutting off the rca plugs and attaching the ground on each channel to the negative output and the hot side of each to the positive, but although it would play for a couple of seconds and sound very good it would cut off after about 2 secs (I assume it was shorting out). Anyway, if anyone knows how to make an adapter that will work or knows where I might buy one, I would appreciate the information.
