Looking for a headphone DAC/amp that also has balanced XLR variable outputs for under $500

I had a Matrix Mini-i that started to randomly shut off. I popped the hood on it and it looks like there's several capacitors that have popped. Not happy about that, and matrix's after sale support was crap. (I had a issue where the amp would  sporadically turn on full blast regardless of volume level once every few months). 
 
Long story short, I really just want to get something good to replace it.
 
So I've been looking for a set of very spacific MUST have features...
-Variable XLR outputs for my desktop powered monitors.
-Built in DAC that can accept both a USB and a optical input.
-Digital or high quality analog volume control. (Reduces/eliminates chances of static during volume changes) 
 
Currently have a pair of Beyerdynamic dt 770 pro 80ohm.

I'm looking at upgrading the Beyer's in the next year or so to a set of Sennheiser hd800 or Beyer Tesla or similar titan class set of cans.
 
I think that $500 should be a reasonable budget to get something really nice...
So, lets have it folks! I've been gone for far too long :D

TEAC UD 301 usually around $350 or less.

That did initially look like a pretty decent DAC, but I've read some issues with the headphone jack not working and with some units the volume knob was a bit staticy. This leads me to believe that the TEAC uses a sub poor analog volume control. Over the years I've found that digital volume controls simply don't have that issue at all. I've edited the initial post to reflect that must have feature.

Sounds to me exactly like the Schiit Jotunheim with DAC

That does look like a contender.

I'm curious if there's anything else out there that would be a good fit for me.
