Looking for a bulk cable with three wires (for TRS, right, left, ground) in it with a max diameter of 4.5mm (preferably 4mm).  
 
I'm trying to use this Neutrik plug.

I don't know of any 3 conductor cables off hand, as usually, for TRS, the shield around a twisted pair is used as ground.  Alternatively, a starquad wire can be used, with two of the wires being used as ground.  Mogami and Canare both make high quality, reasonably priced microphone cables in both configurations that are very popular for DIY cables.
