I can't read it, but it looks like a standard check out form, so I would say:
- Card number
- Expiry date
- CVC
- Cardholder name
/U.
- Card number

Yeah I wasn't sure either. I would suggest copy/pasting it into any online translator (google, babblefish, freetranslations, etc).
