This happened to me once with one of this X10s silicones.  The bride went in the ear on the other side and pulled it through because there was nothing in between to stop it.  Talk about being brainless!
 
Seriously, I change silicones after about 80 hours of use, roughly.  A bit annoying having to have spares on hand, but at least you have the comfort of knowing stuff is not wearing an tearing...and lodging in your ear.
