Headphone jack tip has fallen off

The tip of my headphone jack has fallen off. Here are a few pictures I've taken. http://imgur.com/a/3frRq

The bottom of the tip that is closest to the rest of the jack, has a little thick bit so I can't just push the tip back in with the rest of the jack. I could probably push it in with force/ using a hammer though.
 
How do I repair it? Do I just need to push the tip back in using some force? Will it even work again, or has a wire/ connection been broken already?

Do I need to just change the entire jack?

Replace the whole jack.  You don't want to risk "Fixing" it and having that tip come off inside a player and not be able to get it back out. 
 
You can buy replacements at Parts-express.com (I have no relationship except for customer) for something less than $5.  With 15 minutes and a soldering iron you should be back in business.

Yup, there are plenty of DIY options out there. Amphenol makes a nice value 3.5mm mini, and Switchcraft are pretty well made.

Yep, the other posters are spot on; it's better to replace the whole thing.
 
I would recommend leaving a short length of cable attached to the old plug.  Most times the individual conductors will be color coded.  You can use a multimeter to double check the which color is wired to each conductor on the plug (obviously the tip will be missing, but you can verify the other three).
