Foobar or EAC/LAME problem

I ripped a CD to 256KB/s mp3 in EAC and when I try to play the files in foobar, only some of them work. The working ones have the CDtitle, track number and song length, but the nonworking ones don't. When I try to play the track, it says this:

ERROR (CORE) : error opening file for playback :

with docs and settings and my name in place of the (...). Shoul d I just rip the CD again, or is there something easy I can do?

If you rip again...
why are you using LAME 256 instead of --alt-preset extreme (or --alt-preset fast extreme)? Have something against VBR? Technically you should get slightly higher quality with -apx, while maintaining slightly smaller average sizes.
