custom breakout cable for Apogee Duet 2

Can someone recommend a builder who can put together a custom breakout cable for the Apogee Duet 2?

Here's what I'm looking for :
 
* 2 meters long with higher grade cables
 
* 2 balanced outs with 1/4 termination
 
* 1 1/4 termination stereo input for guitar
 
Is this possible?

This looks like the connector Apogee is using for the Duet 2 :

Unfortunately, I don't know that Apogee has provided a pin-out for that connector, so a builder wouldn't know what wires go where. That makes this rather difficult.
