Chaintech AV-710 (I'm pretty sure that's the model number, do a search for chaintech, it'll come up).

Other possibilities include:
M-Audio Revolution 7.1
AudioTrak Prodigy (but I'm not sure how much these are running)

The Chaintech card is around $25 and is generally considered to be the best card short of the E-MU cards which won't support surround sound.

If you get in touch with Zemo, he can hook you up with E-mu's 0404 for $80!

That's quality right there! But it doesn't do surround like Jasper mentioned.

As for the Chaintech AV-710, its plus is that it has quality 2.1 output ... however, at surround settings, quality is poor. Still awesome for $24!

If you must really have surround sound, M-Audio's Revolution 7.1 and Terratec's Aureon 7.1 are very good as well... Good luck.
