Basically, I have a friend who isn't an audiophile and therefore doesn't really care about sound. However, they would greatly benefit from having custom shells as they are having a really hard time finding iems which are comfortable for them. So what is the cheapest way to get custom shells, ignoring audio quality. Reshelling would work well enough. Preferably with detachable cables (so that they can be replaced should they get damaged). We are in the continental US, so shipping costs are a concern for places like Cosmic Ears or anywhere else outside of the US. Getting something under $200 is more or less the goal, but even cheaper would be better.

Custom tips would be the best priced item and then inearz, com offers customs starting around 200.00. Westone starts off around d 250.00 and there are others that are in the same price category starting for customs.

I'm sorry to post information about the gear that does not meet your price criteria, but have a look at the Alclair Reference triple driver CIEM ($400) and at the corresponding review.
