I have a pair of ATH-M30 that have been through some wear. Most notably the headband is creased in a few spots (the band is intact but the foam is pushed in). How can I avoid that for the future with different headphones? And is it possible to fix?

Also the earpieces (the separator between the drivers and the ears - not the cup) isn't flat anymore. There is air in between them in both ears. Is this possible to fix?

Are these headphones worn out such that I should just buy a new pair?

I mean the part that directly touches the ear not the part that goes around it. The middle of the earpad (the flat part). Hope you know what I mean. I am away right now but I can post a picture when I get home.

Okay, you're just talking about the inner cover of the ear pad. You can easily replace the pads.

There are various other compatible pads you can find on Amazon and so on that are cheaper than the official ones.
 
As for the headband, I don't know how to replace it. Perhaps you could pay Audio-Technica to do that. There are aftermarket headbands and headband cushions you can buy as well. (Just search for them.)
 
To help avoid future issues, you can use a headphone stand and be careful with the headphones in general.

How could the headband could have gotten bent? It sat flat on my desk (no stand). Was it too tight or loose on my head? And if so how can I fix it for my new pair?

I'd need to see photos to have a better idea. It's not uncommon for headbands to become creased over time, even with careful use. It doesn't interfere with normal use of the headphones, so it's more of an aesthetic than practical issue. I'm assuming you have a certain way you like to wear them when they're most comfortable, and that in itself may very well make it crease over time. I'm not sure whether a headband cushion would prevent that...but then again, creases don't matter much to me either way.
